These include a retirement savings plan-401(k), company-paid life insurance, legal assistance, ID theft consult, financial services, short- term and long-term disability benefits and voluntary benefits (accident, hospital and critical illness).

WHO IS ELIGIBLE?
You are eligible to enroll in Smiths Group benefits if you are scheduled to work (20 or more hours per week- beginning August 1, 2023).

Dependents eligible to participate in the plan include:

  • Your legal spouse or registered domestic partner.
  • Children under age 26 (eligibility ends the day they turn 26 (may be eligible for COBRA)
  • Dependent children of any age (including those of a registered domestic partner) who have an eligible disability (approved by Social Security) and are dependent on you for support.

Default Medical Coverage- If you are eligible and do not enroll in a medical plan or waive medical coverage (you have other medical coverage) you (no dependents) will default to the Anthem Advantage HSA Plan and you will only have the opportunity to change/waive coverage if you have a qualified life event or during the next open enrollment, whichever is first.
