When you enroll in an Anthem medical plan through Smiths, you have access to a number of programs to help you stay well, including:

Transform Diabetes Care (TDC)- a CVS Caremark Partner

Only employees that meet certain clinical eligibility criteria will be eligible to enroll in the program and make use of the full resources and services offered by TDC.

What do I need to do?
Nothing,  TDC will be in touch with you if you take diabetes medications or are already enrolled in a diabetes program.  You may opt out at any time.

Sleep Study Program

Sleep is an important part of staying healthy. If you’re not getting enough, or you’re not getting “good” sleep, you’re at risk for more than just being tired. Lack of sleep can increase your risk for serious conditions, such as heart disease, high blood pressure, stroke and even diabetes. The Sleep Management Program, administered through Anthem’s AIM Specialty Health division, includes outpatient and home sleep testing and therapy to help you and your doctor determine how to address issues like sleep apnea and help you get a good night’s sleep. LiveHealth Online- Telehealth (non-emergency care)

Forming a relationship with a doctor in-person is important, but a telehealth service can treat everyday health care issues—any time, from anywhere—saving you time and money.

LiveHealth Online allows you to speak by phone or video with a board-certified doctor 24/7 for a diagnosis of minor, short-term issues, such as:

• Flu /Cold/ fever
• Tooth pain
• Pink eye
• Allergies
• Sore throat
• Skin infections /Minor rashes
• Headache
• Diarrhea

If you are prescribed a medication, prescriptions can be called in to your local pharmacy if you provide that information to the doctor.

You must register before you can use LiveHealth Online. Go to livehealthonline.com to set up your account. You’ll need to provide your name, date of birth, gender, state, ZIP code, email address and your medical insurance ID number. There is no cost for signing up, you pay only $55 for a virtual medical appointment. Your health is too important to ignore.

Hinge Health is more than exercise therapy.  Their virtual programs combine gentle exercise with 1:1 support to reduce your pain and help you move with confidence. Their programs are provided at no additional cost to you and your eligible dependents enrolled in an Anthem medical plan.
Hinge Health provides all the tools you need to get moving again from the comfort of your home. Here are some of the ways your treatment plan could be tailored to you:

● Get a personal care team, including a physical therapist and health coach
● Schedule personal physical therapy sessions as needed
● Receive wearable sensors that give live feedback on your form in the app
If you don’t have pain and are just looking to stay healthy, you can sign up for their free app. Recommended exercises will be tailored to you based on your job and lifestyle.

COEs are top-rated facilities typically with faster recovery times and better clinical outcomes, often at a lower cost. For treatment of other conditions, find an in-network provider or consult your primary doctor about the best course of action.

American Imaging Management- Quality Diagnostic Imaging ( MRI/CT scan)

The AIM Imaging Cost & Quality Program provides you with access to important information about imaging services. If you need a MRI or  CT scan, it’s important to know that costs vary quite a bit depending on where you go to receive the service. Sometimes the differences are significant — anywhere from $300 to $3,000 — a higher price doesn’t guarantee higher quality. When you’re paying a portion of the cost (like your deductible or coinsurance), where you go can make a very big difference to your wallet.

That’s where the AIM Imaging Cost & Quality Program comes in — AIM does the research for you and makes it available to help you find the right location for your MRI or CT scan. Here’s how the program works:

  • Your doctor refers you to a radiology provider for an MRI or CT scan.
  • AIM works with your doctor to help make sure that you are receiving the right test — using evidence-based guidelines.
  • AIM also reviews the referral to see if there are other providers in your area that are high quality but have a lower price than the one you were referred to.
  • If AIM finds another provider that meets the quality and price criteria, AIM will call to let you know.
  • You have the choice — you can see the radiology provider your doctor suggested OR you can choose to see a provider that AIM recommends. AIM will even help you schedule an appointment with the new provider.
