You are required to certify that you and your covered spouse/domestic partner are tobacco free. If you do not, you will be required to pay an annual $600 surcharge for each of you (deducted in equal amounts each paycheck). If you enroll in the LiveHealth Online Tobacco Free Program and complete the coaching sessions, you can eliminate the tobacco surcharge from the date you complete.
