Vision

You may choose between two vision plans, both of which are administered by EyeMed Vision Care and cover nearly all your vision needs from eye exams to glasses and contacts:

  1. The Core Plan – provides one exam, lenses/contacts and/or frames once per 24-month period.
  2. The Enhanced Plan – provides one exam, lenses/contacts and/or frames every 12 months, plus higher frames and contact allowances.

You may use any vision care provider; however, you will receive a higher level of coverage if you use in-network providers and you will not have to file any claims. EyeMed Vision Care’s nationwide select locations include familiar names such as Lenscrafters, Pearle Vision, and Target Optical and thousands of independent doctors. Visit the EyeMed website to confirm whether your provider participates in the network.

The Vision Plans At-A-Glance

Below is an overview of each plan. For more information, see the Summary Plan Description (SPD) for specific details.

Preventive Eye Care

What many people don’t realize is that dental and vision exams are a good way to catch other conditions, like diabetes and high blood pressure, early on, helping you treat them before they become serious, chronic or life threatening.

EyeMed’s vision plans cover an annual eye exam with a $10 copay (Core Plan) or for free (Enhanced Plan) if you see an In-Network provider. This preventive checkup for your eyes can help identify any potential conditions that may be putting your health at risk.

Find an In-Network Provider

Keep your costs lower, and avoid the hassle of filling out paperwork, when you use an in-network provider. Go to www.eyemedvisioncare.com or call 1-866-723-0514 to find network providers near you (Plan IDs: Core Plan: 9681719; Enhanced Plan: 1006166).
