Smiths is preparing to mail Form 1095‑C to all U.S. employees for the 2025 tax year. Forms will be sent by January 31, 2026, to the home address currently on file.
Form 1095‑C is provided to employees who were offered health coverage during the year. Some employees may need this form when completing and filing their tax returns.
Employees who need help obtaining a 1095-C should contact their local HR department for assistance.
